This office issues and maintains birth and death certificates for unincorporated communities only. Facilities under its jurisdiction are the Nassau University Medical Center in East Meadow, South Nassau Communities Hospital in Oceanside and Franklin Hospital Medical Center in North Valley Stream. Villages have their own clerk and vital record offices. There is a $10.00 fee per certified copy requested. Payment may be made in cash or by local check or money order made payable to "REGISTRAR". Credit Card service is available to expedite requests. Office hours are Monday through Friday, 8 a.m. to 4:45 p.m. Please arrive before 4:30 p.m. to allow time for processing.
